Application Forms

Download the applicable forms, and ensure that you have completed them in accordance with Regulations 5 (3) and (4) of the Medicinal Cannabis Industry Act.

Cultivation Licences (Classes B to E ONLY) are issued as a package in combination with, manufacturing, transport and research licences. A separate set of application forms are required for EACH licence.

Each of the following documents must be completed for new and renewal applications.

FORM 1: Licence Application

Section B should be completed by individuals and sole traders ONLY, Section C should be completed by companies and other businesses ONLY and ALL other sections are to be completed by all applicants.

Please sign the Authorisation for Background Checks, the Final Declaration and initial each page of this form.

Ensure that all relevant supporting documentation are attached, then proceed to Sub-Forms A-F.

In completing this form, respond ONLY to the specific Sub-Form related to the licence for which you are applying.

FORM 2: Employee Information List

Form 2 must be completed for each employee using multiple copies of the form if necessary.

FORM 3: Consent of Owner

This form is required ONLY if the applicant does not own the premises(s) and/or motor vehicle(s) being licensed.

The Declaration attached to this form must be signed by the legal (titled) owner(s) of the premises(s) and/ or motor vehicle(s), and the applicant must initial each page of this form.
